Who we are

Practify Health ("Practify", "we") runs practifyhealth.com. This policy covers this website only. The Practify app at app.practifyhealth.com is covered by your agreement with us.

Patient information

This website does not collect patient health information. Do not send it through the contact form. Practify signs a business associate agreement (BAA) with a clinic before it handles any protected health information.

What the contact form collects

• Your name and work email
• Your clinic name, state and number of providers
• Whether your clinic is new or already open
• Your message, if you write one
• A one-way hash of your network address, used to spot abuse. We do not store the address.

The spam check from Cloudflare also reads technical data from your browser to tell people from bots.

Why we use it

We use it to reply to you, to talk about Practify, and to protect the site from abuse. We do not use it for advertising.

Who handles it for us

• Cloudflare hosts this site and runs the spam check.
• Supabase stores the contact form data.
• Resend delivers the email that tells our team about your message.

They handle it only to run these services for us. We do not sell your information. We do not share it for advertising.

Cookies and tracking

There are no advertising or analytics cookies on this site. We load no analytics, ad or social scripts. Fonts come from our own servers. Cloudflare may set a strictly necessary security cookie. For that reason we show no cookie banner.

We honor Global Privacy Control. There is nothing to opt out of, because we neither sell nor share your information.

How long we keep it

We delete contact form data 24 months after your message if we have no business relationship with you by then.

Your rights and how to use them

You can ask to see, correct or delete what we hold about you. Email hello@practifyhealth.com with the email address you used. We reply within 45 days.

Notice for California residents

Under the California Consumer Privacy Act, as amended (CCPA and CPRA), you have the right to know what personal information we collect, to delete it, to correct it, to opt out of its sale or sharing, and to be free of discrimination for using these rights.

In the last 12 months we collected identifiers (name, email, a hashed network address) and professional information (clinic name, state, number of providers) from the contact form. We collected no sensitive personal information. We did not sell or share personal information. You may use an authorized agent to make a request.

Notice for Texas residents

Under the Texas Data Privacy and Security Act, Texas residents may confirm that we process their data, access it, correct it, delete it, get a copy of it, and opt out of targeted advertising, sale and profiling. We do none of those. If we deny a request, you may appeal by replying to our answer. We respond to appeals within 60 days.

Children

This site is for clinic owners and staff. It is not for children, and we do not knowingly collect information from anyone under 16.

Changes

We may update this policy. The date at the top shows the latest change.
